History: Athlete had history of knee trauma, ruptured ACL, and chronic knee pain with cycling. Cyclist was unable to push on hills or sprint due to the pain, and was thus not able to race.
Exam Findings: Musculoskeletal exam found significant pronation and associated femoral internal rotation with gait, both on and off the bike. Sample orthotics placed in the cycling shoes minimized lateral deviation of the knee with cycling.
Diagnosis, Treatment, Fitting: The saddle and cleats were adjusted to minimize lateral deviation at the knee with cycling. This improved his overall hip/knee/foot alignment. Patient was casted for custom orthotics.
Follow Up: The custom orthotics were fitted to the cycling shoes and the athlete was observed pedaling with the orthotics in the shoes. The orthotics significantly improved the alignment and motion of both legs. The athlete reported resolution of his pain and was able to race his bike again without pain.

CONCLUSION
The athlete had a medically significant injury history that had a significant impact on the fitting process. Hosmer Chiropractic was able to make recommendations for this complex case that allowed the athlete to resume training and racing without restriction.
